چکیده

Hypertensive encephalopathy is an acute neurological syndrome caused by a sudden increase of systemic blood pressure (1, 2). The imaging abnormalities are predominant in the parieto-occipital subcortical white matter. The clinical and radiological abnormalities are reversible after antihypertensive treatment (3). Therefore, hypertensive encephalopathy has been called posterior reversible encephalopathy syndrome (PRES). Brainstem was involved in 58% of hypertensive encephalopathy in one study (4) but isolated brainstem involvement of hypertensive encephalopathy has been rarely reported (5-9). In this case, rare isolated hypertensive brainstem encephalopathy was occurred with intracerebral hemorrhage (ICH). ICH is more common acute neurologic complication of systemic hypertension and may be an indicator of hypertensive attack in hypertensive encephalopathy. On the other hand, acute elevations of intracranial pressure (ICP) due to ICH can cause a further increase of systemic hypertension and an increased intracerebral capillary pressure and permeability (10, 11).

Pheochromocytoma is a rare cause of hypertension in children. Hypertension is one of the common reasons of posterior reversible encephalopathy. Intracerebral hemorrhage is a serious and unexpected complication of hypertensive encephalopathy due to pheochromocytoma, and very rarely seen in the childhood. Intracerebral hemorrhages should be searched if there are hypertensive reversible signal cha...

Isolated Brainstem Involvement in a Patient with Hypertensive Encephalopathy

Hypertensive encephalopathy typically presents with headache, confusion, and bilateral parietooccipital vasogenic edema. Brainstem edema in hypertensive encephalopathy usually occurs in association with typical supratentorial parieto-occipital changes and is usually asymptomatic. We report here a patient with hypertensive encephalopathy, with isolated brain stem involvement on magnetic resonanc...

Hypertensive Encephalopathy with Reversible Brainstem Edema

Presented here is a 36-year-old male with arterial hypertension who developed brainstem edema and intracranial hemorrhage. Magnetic resonance scan revealed diffuse brainstem hyperintensity in T2-weighted and fluid-attenuated inversion-recovery images, with an increase in apparent diffusion coefficient values. After a reduction in blood pressure, rapid resolution of the brainstem edema was obser...
